29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: Maintenance Shop Area, 80 State St Rear, North Adams, MA On 12/14/12 and at times prior, the employer had not instituted energy control procedures for the P & H Mobile Welder. On 12/14/12, an employee diagnosing an engine problem removed the engine cowling while the engine was operating to adjust the throttle linkage by hand. The employee's hand contacted the rotating cooling fan resulting in a serious hand injury.
