29 CFR 1910.215(a)(4): Work rest(s) on grinding machinery were not adjusted closely to the wheel with a maximum opening of one eighth inch: a) Jet Bench Grinder: On January 15, 2013 and at times prior thereto, employees were exposed to struck by hazards from the tool rest being approximately three eighths of an inch from the abrasive wheel. Material can become stuck between the tool rest and the abrasive wheel causing the abrasive wheel to break and striking employees.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch: a) Jet Bench Grinder: On January 15, 2013 and at times prior thereto, employees were exposed to struck by hazards from the tongue guard being approximately two inches from the abrasive wheel. If the abrasive wheel were to break, pieces of the wheel would strike employees. b) Delta Bench Grinder: On January 15, 2013 and at times prior thereto, employees were exposed to struck by hazards from the tongue guard being approximately one and one eighth of an inch from the abrasive wheel. If the abrasive wheel were to break, pieces of the wheel would strike employees.
