29 CFR 1910.212(a)(2): Machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points and rotating parts: (a) On January 17th, 2013, employees of Milbank Manufacturing were exposed to caught in hazards from exposed rotating parts on a DoAll drill press located in the maintenance area. The guard for the machine was lying on the ground behind the machine. (b) On January 17th, 2013, employees of Milbank Manufacturing were exposed to caught in hazards from exposed rotating parts of a Procunier Drill press in the top cell area.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) On January 17th, 2013 employees of Milbank Manufacturing were exposed to contact with electrical current hazards when flexible cords were used throughout the facility for permanent wiring. Many of the cords were also permanently affixed to various parts the building with zip ties increasing the likelihood of damage to the cord insulation.

(a) On January 17th, 2013 employees of Milbank Manufacturing were exposed to contact with electrical current and fire hazards when one 120 VAC and one 480 VAC electrical panels were blocked by machinery and other various materials in the maintenance and welding areas.
