29 CFR 1926.405(a)(2)(ii)(I): Protection was not provided to avoid damage to flexible cords and cables used for temporary wiring which passed through doorways or other pinch points: (a) Rear exit door on south side of Church located at 940 Concord Road, Smyrna, GA 30082: Employees using an 50' orange extension cord inside the building did not protect the cord from becoming pinched between the metal door jamb and the metal door causing a possible exposure of energized circuits. Employees using this cord to power a Makita (Model 5007F) 7 1/4" Circular Saw and a microwave were exposed to shock hazards when accessing the door.

The manufacturer of the ladder stated that it should not be used as a personnel ladder.
