29 CFR 1926.102(a)(1): Eye and/or face protective equipment were not used when machines or operations presented potential eye or face injury:         a) On or about January 16, 2013 at 160 Stone Park Blvd, Pike Road, Al., The employer failed to ensure employees using nail guns and circular saw while nailing / trimming first floor decking material wore eye protection.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) On or about January 16, 2013 at 160 Stone Park Blvd, Pike Road, Al: The employer failed to ensure employees trimming the edge of the first floor decking and installing walls of a residential building used fall protection which exposed them to falls of 8 feet, 4.5 inches to the ground below.
