Section 5(a)(1) of the Occupational Safety and Health Act of 1970:  The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that the employees were exposed to being struck by a truck:     a) 80 Leland Avenue, northeast corner, on or about 1-21-13, One employee was operating a recycling/trash collecting truck during unloading operations, when the truck rolled forward, striking the employee and pinning him under the truck.        Among other methods, a feasible method of abatement to correct this hazard is to:         1)  Train all employees who will be operating refuse/recycling collection vehicles and equipment, including but not limited to compliance with the relevant provisions of American National Standards Institute (ANSI) Z245.1-2008, for Mobile and Recycling Materials Collection, Transportation, and Compaction Equipment-Safety Requirements, such as:      Clause 6.1.3.2  Training Criteria; incorporating training requirements into a safety program, as specified in clause 9 of this standard, utilizing the information provided by manufacturers and employers according to clause 5 and 6 of this standard;      Clause 9.3.3  Training shall include a practical demonstration of equipment operation knowledge and skills by the employee, as appropriate to the requirements of the employee's job duties.     Clause 9.3.4  Training shall include at a minimum; (c) Operational instruction on each specific type of equipment used by the employee, including provisions contained in clause 6, (Operational Requirements), 7 (Mobile Equipment safeguards and Features), and 8 (Minimum Requirements for secondary drive provision), and the information by the manufacturer pursuant to clause 5 (Instructions for Operations and Maintenance) of this standard.     Allison Transmission Operator's Manual. 1000 and 2000 families, 2003 June, Revision 1-2004 July, OM3063EN; states to place the vehicle in neutral, set the parking brake, and chock the rear wheels when the operator's cab is unoccupied with the engine running.
