29 CFR 1904.4(a): The employer did not record each work-related fatality, injury or illness case that resulted in the general recording criteria on the OSHA Form 300 or equivalent. At the facility: the employer did not record the following workplace injuries on the OSHA Form 300 or equivalent for the year 2012: On 8/27/12, an employee cut his finger. He went to the hospital where he received stitches. He then returned to work where he was able to perform his normal duties.

29 CFR 1910.303(g)(1): Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment: Lower Brown Building Garage - Working space in front of electrical panel FS-1 was not maintained as it was being used for storage of a ladder and other miscellaneous items such as wire for welding and electrical cords.
