29 CFR 1910.303(g)(1)(iii): The employer did not ensure that at least one entrance of sufficient area was provided to give access to the working space about electric equipment 600 Volts, nominal, or less. (a) Maintenance Shop: There was not an adequate entrance to the electrical panel box as the entrance was only 13 inches wide as it was narrowed by a peg board, a box on the floor, and the two storage racks.

29 CFR 1910.305(g)(1)(iv): Flexible cords and/ or cables were used for purposes prohibited by paragraphs (A) through (F) of 29 CFR 1910.305(g)(1)(iv) (a) Line 2216: Employees were exposed to electrical wiring for the light for the Anilox Rack which was subject to abrasion. The wiring for the light was run through the wall between Line 2216 and Line 2204. The light was on Line 2216, and the outlet was located on Line 2204 where the cord was plugged in.

29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: (a) Shipping/Rewind Area: There was not adequate strain relief for the pendant cord attached to the electrical duplex box. The electrical cord where it attached to the duplex box, the insulation had pulled back exposing interior wiring.
