29 CFR 1910.147(d)(4)(i): Lockout or tagout devices shall be affixed to each energy isolating device by authorized employees. The employer did not insure that employees were protected from accidental startup when they did not use an isolating blocking device to lock out the breaker switch for the Amada horizontal band saw, S/N 40600629, when doing servicing and maintenance on the saw, located in the shop area of KFF Supply, Centralia, IL.

29 CFR 1910.243(c)(4): The top half of the wheel of portable grinding machine(s) was not enclosed: Two Milwaukee portable angle grinders, 7.5 amp capacity, located in the shop area of KFF Supply in Centralia, IL, used to knock off slag from piping, did not have guards covering 180 degrees of the 4-1/2 inch abrasive disk, exposing employees to cuts and caught-in hazards during use.

Yellow extension cord used to power the Milwaukee grinders showed insulation strain at the plug end, exposing employees to electric shock hazards.
