29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative: a.) Davis Fabricators, Inc. located at 15765 W. St. Route 2, Oak Harbor, OH 43449; On or about January 23, 2013, the employer did not assure an energy control program consisting of employee training, periodic inspections, and machine specific procedures, was established. Employees were exposed to caught by and amputation hazards while performing setup and maintenance activities on equipment including, but not limited to; the Standard Press Brake (Model# AB200-12), Marvel Band Saw (Model#81A11PC), Gardner Belt Sander, and the Roundo Angle Rollers.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a.) Davis Fabricators, Inc. located at 15765 W. St. Route 2, Oak Harbor, OH 43449; On or about January 23, 2013, the employer did not assure employees were protected from amputation hazards from the Marvel Band Saw (Model#81A11PC). Employees use the band saw for various fabricating activities and are exposed to amputation hazards from the unused portion of the blade. b.) Davis Fabricators, Inc. located at 15765 W. St. Route 2, Oak Harbor, OH 43449; On or about January 23, 2013, the employer did not assure employees were protected from caught by hazards from the Jet Drill Press (Model #700DS). Employees use the drill press for various milling activities and are exposed to caught by hazards from the unguarded chuck. c.) Davis Fabricators, Inc. located at 15765 W. St. Route 2, Oak Harbor, OH 43449; On or about January 23, 2013, the employer did not assure employees were protected from caught by hazards from the Gardner Belt Sander 3CB. Employees are exposed to unguarded rotating parts and ingoing nip points when working on and around the sander.

Employees were exposed to struck by and caught by hazards while using the grinders on metal parts.
