29 CFR 1910.176(b):     Secure storage. Storage of material shall not create a hazard. Bags, containers, bundles, etc., stored in tiers shall be stacked, blocked, interlocked and limited in height so that they are stable and secure against sliding or collapse.      Storage Area - The pallets of box cartons were not stacked and limited in height so that they were stable and secure against sliding and collapse.

29 CFR 1910.212(a)(1):   Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.      a)   Maintenance Dept. - The Grizzly drill press was not provided to protect the operator from the rotating chuck and drill bit.    b)   30 Pound Packing Line - The infeed and outfeed of the transfer conveyor had inrunning nip points between the chain and sprocket that were not guarded.    c) Inspection Room - The feed conveyor for the inspection lines had inrunning nip points and rotating spoke sprocket that were not guarded.    d) Inspection Room - The inspection lines had rotating spoke sprockets or solid sprockets with ribs at the outfeed end of the conveyor that were not guarded.    e) Inspection Room -  The infeed end of the discharge conveyor had inrunning nip points and rotating solid sprockets with ribs that were not guarded.

29 CFR 1910.219(c)(2)(i):   All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires.    a) 30 Pound Packing Line - The exposed rotating shafts at each end of the conveyor were not enclosed.    b) Uncuffer Feed Conveyor - The two horizontal rotating shafts were not enclosed.    c) Inspection Room - The horizontal rotating shaft for the feed conveyor to the inspection lines was not enclosed.    d) Inspection Room - The horizontal rotating shaft for the discharge conveyor was not enclosed.    e) Inspection Room - The horizontal rotating shafts for the outfeed end of the inspection lines were not enclosed.    e) Metal Detector Table - The horizontal rotating shaft for the outfeed end was not enclosed.

Packing Area - The new electrical panel did not have an arc flash warning sign.
