29 CFR 1910.22(a)(3): To facilitate cleaning, every floor, working place, and passageway shall be kept free from protruding nails, splinters, holes, or loose boards. (a) Rightside Warehouse- On or about January 16, 2013 wood boards and wood boards with nails fell from the ceiling and stuck an employee. On or about January 22, 2013 wood boards and wood boards with nails were observed on the work floor creating a fall hazard for employees.

Each service, feeder, and branch circuit, at its disconnecting means or overcurrent device, shall be legibly marked to indicate its purpose, unless located and arranged so the purpose is evident. (a) Rightside warehouse- On or about January 22, 2013 employees were exposed to electric shock hazard due to a circuit breaker box not being labeled for emergency shut-off. This main circuit breaker boxes labels were washed off because of exposure to rain water form leaks in the building roof.
