29 CFR 1926.451(h)(2)(ii): A toeboard was not erected along the edge of platforms more than 10 feet (3.1 m) above lower levels for a distance sufficient to protect employees below:   a) Second buck of scaffold at front face of home;  On or about January 23, 2013 and at times prior thereto, the employer exposed his employees to struck-by hazards in that stone was placed on the edge of the platform without a toeboard which exposed employees working below to being struck-by by falling stone.    b) Third buck of scaffold at front face of home;  On or about January 23, 2013 and at times prior thereto, the employer exposed his employees to struck-by hazards in that stone was placed on the edge of the platform without a toeboard which exposed employees working below to being struck-by by falling stone.

29 CFR 1926.451(c)(2): Supported scaffold poles, legs, posts, frames, and uprights did not bear on base plates and mud sills or other adequate firm foundation:  a)  Bracket scaffold at front-face of home;  On or about January 23, 2013 and at times prior thereto, the employer exposed his employees to a 15.7' fall hazard in that employees were allowed to work from a bracket scaffold where the scaffolding was erected on mudsills without all the required base plates.    b)  Second buck of scaffold at front-face of home;  On or about January 23, 2013 and at times prior thereto, the employer exposed his employees to a 13' fall hazard in that employees were allowed to work from the second buck of of a three buck high scaffold where the scaffolding was erected on mudsills without all the required base plates.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used:  a)  Bracket scaffold at front-face of home;  On or about January 23, 2013 and at times prior thereto, the employer exposed his employees to a 15.7' fall hazard in that employees were allowed to climb up the side supports of a scaffold without the use of a ladder or other means of safe access.  Employees used the side supports to access the bracket scaffold.  b)  Second buck of scaffold at front-face of home;  On or about January 23, 2013 and at times prior thereto, the employer exposed his employees to a 13' fall hazard in that employees were allowed to climb up the side supports of a scaffold without the use of a ladder or other means of safe access.  Employees used the side supports to access the second buck of the scaffold.  c)  First buck of scaffold at east side of home;  On or about January 23, 2013 and at times prior thereto, the employer exposed his employees to a 6' fall hazard in that employees were allowed to climb up the side supports of a scaffold without the use of a ladder or other means of safe access.  Employees used the side supports to access the first buck of the scaffold.
