29 CFR 1910.27(d)(1)(iv) Cages did not extend down fixed ladders to a point not less than 7 feet nor more than 8 feet above the base of the ladder.    (a)    On, or on dates before, May 30, 2013, employees were exposed to fall hazards because a cage on a fixed ladder attached to an outdoor dry products elevator ended approximately 14 feet above the base of the ladder at ground level.
