29 CFR 1926.502(d)(17): The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head: Employees re-roofing the apartment complex at 7901 E. Lincoln St., Wichita, Kansas were exposed to fall hazards of 16 feet 6 inches to the concrete parking lot. An employee had tied the anchorage rope off to the side of his harness with a knot.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards: Employees re-roofing the apartment complex at 7901 E. Lincoln St., Wichita, Kansas were exposed to fall hazards of 16 feet 6 inches to the concrete parking lot. Employees were not trained in the proper use of fall protection in that employees did not assemble the personal fall arrest system correctly.
