29 CFR 1910.132(a): The employer did not ensure Protective equipment, including personal protective equipment for protective shields and barriers, shall be provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact: On or about January 18, 2013, and at times prior thereto, employees working on the edge of a flat structure while changing lights and conducting electrical work were exposed to a fall of approximately 19 feet 4 inches to the ground below while protective equipment was not used when necessary whenever hazards capable of causing injury and impairment were encountered.
