29 CFR 1910.132(a): Protective equipment, including personal protective equipment for eyes, face, head, and extremities, protective clothing, respiratory devices, and protective shields and barriers, were not provided, used, and maintained in a sanitary and reliable condition wherever it is necessary by reason of hazards of processes or environment, chemical hazards, radiological hazards, or mechanical irritants encountered in a manner capable of causing injury or impairment in the function of any part of the body through absorption, inhalation or physical contact:    This violation was discovered on January 23, 2012, at a workplace located at 16576 Highway U.S. 79, Jacksonville, Texas 75766, where the employer allowed at least one (1) employee to work from the top of an unfinished 3,000 gallon water tank that was more than 4-feet in height, without providing the employee with personal protective equipment.
