29 CFR 1926.451(g)(3)(i): Where vertical lifelines are used, the employer did not ensure that they were protected from sharp edges and abrasions: (a) Universal Limited at 3200 RBC Road 31, Rifle, Colorado 81650: On and before January16, 2013, the employer did not ensure that each employee's lifeline was protected from sharp edges and abrasions. Employees were installing metal siding on a building from a suspended scaffold and draped their lifelines over a horizontal member of the building just below the anchorage points, thus exposing the lifelines to abrasion and reducing the strength properties of the lifeline. Condition exposed employees to the hazard of fall from elevation at an approximate height of 25 feet or more.

29 CFR 1926.502(d)(15): Anchorages used for attachment for personal fall arrest equipment were not capable of supporting at least 5,000 pounds per employee: (a) Universal Limited, Inc. at 3200 RBC Road 31, Rifle, Colorado 81650: On and before January16, 2013,the employer did not ensure that each employee was protected by the use of a complete fall protection system meeting the requirements of paragraph (a)(2) of S 1926.502 of this Subpart, in that employees were installing metal siding on a building from a suspended scaffold and did not use anchorage points capable of supporting at least 5,000 pounds per employee attached. Employees anchored their rope grab system to the structural steel I-beam of the building by wrapping the lifelines around the beam and tied back to themselves with a knot, thus reducing the strength properties of the lifeline. Condition exposed employees to the hazard of fall from elevation at an approximate height of 25 feet or more.
