29 CFR 1910.106(b)(6): In locations where flammable vapors may be present, precautions were not taken to prevent ignition by eliminating or controlling sources of ignition.  Sources of ignition may include open flames, lightning, smoking, hot surfaces, frictional heat, sparks (static, electrical, and mechanical), spontaneous ignition, chemical and physical-chemical reactions, and radiant heat:    (a) On or about January 18, 2013, and at times prior, for employees performing "glassing" work, where styrene resin is mixed with a catalyst and applied to fiberglass sheets in order to replace leaking fittings inside fiberglass tanks and an L.B. White Tradesman kerosene heater with an open flame was used in order to heat the materials so that they "kick," i.e., cure and harden, in fiberglass tanks at the H State well 1-36-25H and other oil and gas locations throughout North Dakota.

29 CFR 1910.146(d)(3)(iv): Under the permit space program, the employer did not develop and implement the means, procedures, and practices necessary for safe permit entry operations, including, but not limited to, purging, inerting, flushing or ventilating the permit spaces as necessary to eliminate or control atmospheric hazards:    (a) On or about January 18, 2013, and at times prior, where a flammable atmosphere was allowed to develop while employees performed "glassing" work, whereby styrene resin is mixed with a catalyst and applied to fiberglass sheets in order to replace leaking fittings inside a fiberglass product water tank at the H State well 1-36-25H and other oil and gas locations throughout North Dakota.

White Tradesman kerosene heater with an open flame was used to heat the materials so they would "kick," i.e., cure and harden, in a fiberglass product water tank at the H State well 1-36-25H near Grenora, North Dakota, and other oil and gas locations.    Note:  This company was previously cited for a violation of this Occupational Safety and Health standard or its equivalent standard which was contained in OSHA Inspection Number 111117, Citation 1, Item 3C, and was affirmed as a final order on July 17, 2012, with respect to a workplace located at 7600 Highway 2 East in Minot, North Dakota.
