(a) Intelagard, 1275 Rock Creek Circle, Lafayette, CO 80026; On and before January 23, 2013, the exposing employer failed to place an adjustable tongue guard on a table top grinder located in the rear of the main machinery shop. Condition exposed employees using the grinder to eye hazards from flying particles or portions of the item being grinded.

29 CFR 1910.253(b)(2)(iv): Valve protection caps, where cylinder is designed to accept a cap, were not kept in place, hand-tight, except when cylinders were in use or connected for use. (a) Intelagard, 1275 Rock Creek Cir., Lafayette, CO 80026; On and before January 23, 2013, the exposing employer failed to ensure that an oxyacetalene welding rig which had cylinders ready to accept valve protection caps, had those caps in place. Condition exposed the welding rig to possible damage from accidental contact to the valves and thus employees to possible fire and explosive hazards.
