29 CFR 1926.405(g)(2)(iii):     The repair splice in a hard service flexible cord No. 12 or larger did not retain the insulation, outer sheath properties, and usage characteristics of the cord which was being used:    At this worksite, employees used cord and a  plug portable hand tool connected by an extension cord to the power supply pole and the extension cord had been inadequately repaired.

29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    At this worksite, employees worked from the top of scaffolds where the walking/working platforms were not fully planked or decked such as but not limited to the following:    a.  The two tiered scaffold on the west end of the building where employees worked at a height of twelve and one-half feet above the concrete ground level.    b.  The five tiered scaffold on the east end of the building where employees worked at a height of thirty-five feet above the concrete ground level.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Cross braces were used as a means of access.    At this worksite, employees worked from scaffolds at heights greater than ten feet and the employer did not ensure safe access and egress such as but not limited to the following:    a.  The two-tiered scaffold on the west end of the building; and    b.  The five-tiered scaffold on the east end of the building.
