29 CFR 1960.8(a): 29 CFR 1960.8(a):  The Agency head did not furnish employment and a place of employment to each employee that were free from recognized hazards that caused or were likely to cause death or serious physical harm:    a) LSD Garrison Mobile Equipment Branch repair shop, Building 5400 - On or about, and at times prior to February 25, 2013, employees were allowed to use battery chargers that were not installed and used per manufacturer's safe operating procedures.  The battery chargers were attached to wooden pallets (combustible material), thus exposing the employees to life safety hazards such as fire and smoke inhalation.      b) LSD General Warehouse Storage, Building 1330, Bay 3 - On or about, and at times prior to February 26, 2013, employees were allowed to use battery chargers that were not installed and used per manufacturer's safe operating procedures.  The battery chargers were attached to a wooden pallet (combustible material), thus exposing the employees to life safety hazards such as fire and smoke inhalation.      c) LSD General Warehouse Storage, Building 1330, Bay 5 - On or about, and at times prior to February 26, 2013, employees were allowed to use battery chargers that were not installed and used per manufacturer's safe operating procedures. 29 CFR 1910.37(a)(4): Safeguard(s) designed to protect employees during an emergency (e.g., sprinkler systems, alarm systems, fire doors, exit lighting), were not in proper working order at all times:    a) General Storage Warehouse Building 1230, Bay 4, middle support column - On or about, and at times prior to January 26, 2012, the employer did not ensure that a designated exit sign had the capability to be illuminated by a reliable light source, thus employees were exposed to a life safety hazard.        b) General Storage Warehouse Building 1230, Bay 4, back door - On or about, and at times prior to January 26, 2012, the employer did not ensure that a designated exit was illuminated or had the capability to be illuminated by a reliable light source, thus employees were exposed to a life safety hazard. 29 CFR 1910.178(l)(3)(i)(A): The employer did not provide training that included operating instructions, warnings, and precautions for the types of truck the operator was authorized to operate:    a) LSD Garrison Supply and DSSC General Warehouse Storage, Building 1330 - On or about, and at times prior to February 26, 2013, the agency did not ensure employees who operated electric powered forklifts in bay 3 and 5 were trained in accordance with the standards, thus exposing employees to ejection, crush-by and struck-by hazards      ABATEMENT CERTIFICATION IS REQUIRED.
