29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a) MCCS Bowling Center, 90XLi Automatic Pinspotters - On or about, and at times prior to February 20, 2013, the agency did not implement adequate lockout procedures per the manufacturer's safety procedures and precautions, in that the employees were allowed to work on the adjacent (paired) pin spotter while energized, exposing the employees to caught-in, crush-by, and amputation hazards ABATEMENT CERTIFICATION AND DOCUMENTATION ARE NOT REQUIRED

29 CFR 1910.219(e)(1)(i): Horizontal belts which had both runs 42 inches or less from the floor level were not fully enclosed by guards conforming to requirements specified in 29 CFR 1910.219(m) and (o): a) MCCS Maintenance Shop, Building 5592 On or about, and at times prior to February 19, 2013, the agency did not ensure the horizontal belt was adequately guarded on the TS2412, RIGID-10-inch belt drive table saw, thus exposing employees to caught-in hazard. ABATEMENT CERTIFICATION IS NOT REQUIRED
