29 CFR 1910.29(a)(4)(ii):  Ladder stands did not have at least two (2) of the four (4) casters locked to prevent movement:    a)  Warehouse, 3382 Industrial Boulevard, Bethel Park, Pennsylvania, 15102:  Observed on or about January 24, 2013, three Cotterman brand ladder stands used in the Warehouse were not equipped with a means to prevent horizontal movement.  The locking mechanism on each ladder stand was kept open by the use of a wooden stake.  Employees using the defective ladder stands to retrieve automotive parts from industrial storage racks were exposed to a fall hazard of approximately 9 feet to the concrete floor below.
