29 CFR 1926.451(b)(1):     Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):    On or about January 23, 2013, at the worksite located at 106 Tournay, Little Rock, Arkansas, on the supported scaffold located at the front of the home being built, the 2nd level platform that employee worked from installing fascia was not decked the full width of the scaffold.  This condition exposed employee to an approximately 13 ft. fall from elevation.

29 CFR 1926.451(b)(5)(i):     The end of a platform 10 feet or less in length, extended over its support more than 12 inches:    On or about January 23, 2013, at the worksite located at 106 Tournay, Little Rock, Arkansas, on the supported scaffold located at the front of the home being built, the 2nd level platform that employee worked from installing fascia extended approximately 20 inches over its support.  This condition exposed employee to an approximately 13 ft. fall from elevation.

29 CFR 1926.451(e)(1):     When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, sturcure, personnel hoist, or similar surface was not used:    On or about January 23, 2013, at the worksite located at 106 Tournay, Little Rock, Arkansas, on the supported scaffold located at the front of the home being built, employee dimounted from the 2nd level platform on climbed down the end frames to lower ground level.  This condition exposed employee to slips and falls from elevation.

29 CFR 1926.451(g)(1):     Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    On or about January 23, 2013, at the worksite located at 106 Tournay, Little Rock, Arkansas, on the supported scaffold located at the front of the home being built, guardrails were not provided on all open sides and ends to protect employee working from the 2nd level platform installing fascia.  This condition exposed employee to an approximately 13 ft. fall from elevation.

29 CFR 1926.451(f)(3):     Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift, and after any occurrence which could affect a scaffold's structural integrity:    On or about January 23, 2013, at the worksite located at 106 Tournay, Little Rock, Arkansas, the supported scaffold located at the front of the home being built was not inspected for hazardous conditions prior to use.  Hazards such as but not limited to unstable footings, improper decking, lack of fall protection and hollow core masonry blocks being used to weight scaffold were observed.  These conditions exposed employee to the hazard of falls from elevation and/or scaffold collapse.

29 CFR 1926.454(a):     The employer did not have each employee who performed work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used and to understand the procedures to control or minimize those hazard:    On or about January 23, 2013, at the worksite located at 106 Tournay, Little Rock, Arkansas, for the benefit of employees working on supported scaffolds, employees had not been trained to recognize, avoid and minimize potentially hazardous conditions associated with scaffolds such as but not limited to proper use of scaffolds, proper guarding methods for scaffolds, proper decking for scaffolds, proper methods for accessing scaffold platforms and fall hazards.
