Metal Grating on Floor Drainage System for Bay's # 1, 2, 3, and 4 - On or about January 28, 2013, several sections of the metal grating system for the floor was not kept free from protruding metal pieces, missing grating sections, loose grating sections, and holes. Employees were thereby exposed to the hazards of slips, trips, and falls.

29 CFR 1910.23(c)(1): Every open-sided floor or platform 4 feet or more above adjacent floor or ground level shall be guarded by a standard railing (or the equivalent as specified in paragraph (e)(3) of this section) on all open sides except where there is entrance to a ramp, stairway, or fixed ladder. The railing shall be provided with a toeboard wherever, beneath the open sides, a. Working Platform between Bay's # 1 and 2 - On or about January 28, 2013, employees were required to perform tank truck washing operations from a platform above 4 feet from the ground level. The west side section of the platform was not provided with a standard guard railing (missing top rail and mid rail). Employees were thereby exposed to fall hazards. b. Working Platform between Bay's # 3 and 4 - On or about January 28, 2013, employees were required to perform tank truck washing operations from a platform above 4 feet from the ground level. The west side section of the platform was not provided with a standard guard railing (missing top rail and mid rail). Employees were thereby exposed to fall hazards.

Work Light Fixture for Bays # 3 and # 4 - On or about January 28, 2013, employees were required to utilize a portable work light for hazardous conditions (metal guard, 25 ') which had an extension cord plug that was missing its grounding pin. Employees were thereby exposed to electrical hazards.
