On or about January 24, 2013, employees were exposed to struck-by hazards when the employer did not periodically audit the lockout procedures to insure that they were being followed and did not need modifications.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    On or about January 24, 2013, employees are exposed to struck-by hazards when the in-going nip points on the pullers and punch/cut machines were not completely guarded.     a. Large pullers with tractor feed: cardboard guards did not completely cover the tractor feed and were easily moved or damaged; tractor feed could also be accessed under the side guards which were mesh or plywood.   b. Small pullers with belt feed:  paper clip guards were not installed to completely cover the openings and did not prevent access to the bottom of the belt feed; belt feed could also be accessed under the mesh guard.   c. Punch/cut machines: paper clip guards on belt puller were not installed to completely cover the openings.   d. Cutting machine: index feed with no guard to limit access to nip point.
