29 CFR 1926.451(g)(1): Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:    a)  At 26651 Perdido Beach Blvd Orange Beach, Alabama:  On or about January 23, 2013 and at times prior; the employer failed to ensure employees working from a scaffold were provided with fall protection exposing them to falls of 15 feet to the ground below.

29 CFR 1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and fuardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system.    a)  At 26651 Perdido Beach Blvd Orange Beach, Alabama:  On or about January 23, 2013 and at times prior; the employer exposed his employees to fall hazards of 12.7 feet to the ground below in that he failed to provide fall protection for employees working on the roof installing lights.
