29 CFR 1926.153(o): When damage to LP-Gas systems from vehicular traffic was a possibility, precautions against such damage were not taken:    (a) Jobsite, Oak Hill Commons, Pittsburgh, PA:  On or about 01/18/13, a 1000 gallon tank of LP-gas was not protected from vehicular traffic.  The tank was located approximately 20 feet away from the building and there were visible tracks and ruts worn into the ground approximately 2 feet from the tank.  The masonry contractor's lull travels in this area to bring supplies and materials to the work area.  There is no barricade or other precautions taken to protect the tank from potential vehicle damage.  In addition, the supply hose from the tank to the building ran across the path of travel where the lull passed and was buried and frozen into the ground.
