29 CFR 1928.57(b)(1)(ii): All tractors were not equipped with an agricultural tractor master shield on the rear power take-off: On or about January 23, 2013, the employer did not assure all tractors were equipped with an agricultural tractor master shield on the rear power take off. The Ford 3910 agricultural tractor used with Savage Equipment Company 8261 Harvester did not have a master shield on the rear power take off. Employees were exposed to the hazard of amputations.

29 CFR 1928.57(b)(2)(ii): All revolving shafts, including projections such as bolts, keys, or set screws, were not guarded, except smooth shaft ends protruding less than one-half the outside diameter of the shaft and its locking means: On or about January 23, 2013, the employer did not assure the guard was in place on the driveshaft of the Savage Equipment Company 8261 Harvester. Employees were exposed to the hazard of amputations.
