On or about 24 January 2013, and at times prior thereto, the employer allowed three (3) cylinders of Oxygen, two (2) cylinders of Acetylene, and various small, portable, plastic gasoline cans to be stored together in a locked cage along the southern, outside exterior of the main shop building, and within 5 to 10 feet from the propane tank storage area, exposing the employee working within the building or that traverse the area near the cage to a fire, explosion, and struck by hazards.    Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the date of this citation, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure flammable gases and liquids are stored at least 20 feet away from oxidizers, or that a fire-resistant barrier rated to at least one-half hour is located between the two storage areas.

Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment.    On or about 24 January 2013, and at times prior thereto, an electrical breaker panel that provides power to the Kuraki KBT-15DX Horizontal Boring/Milling tool had various items and materials stacked or stored in front, blocking ready access to it for operating the breakers, exposing employees to a struck by hazard or a slip, trip or fall hazard. This panel would be accessed in the event of a breaker tripping or for de-energizing the connected tool for maintenance with a lock-out and tag-out program.
