In apartment 106 leading to the courtyard where employees plugged in extension cords, an electrical panel box containing live electrical wiring and circuit breakers was exposed, thereby exposing employees to a contact hazard.

Unless necessary for a use permitted in paragraph (g)(1)(i) of this section, flexible cords and cables shall not be used where run through doorways, windows, or similar openings, except as permitted in paragraph (a)(2)(ii)(1) of this section. a. On the second floor leading from the courtyard to the apartments on the 2nd floor, an extension cord was run through a sliding glass door, exposing employees to a contact hazard.

29 CFR 1926.404(b)(1)(iii)(C): Each cord set, attachment cap, plug and receptacle of cord sets, and any equipment connected by cord and plug, except cord sets and receptacles which are fixed and not exposed to damage, shall be visually inspected before each day's use for external defects, such as deformed or missing pins or insulation damage, and for indications of possible internal damage. Equipment found damaged or defective shall not be used until repaired. a. In apartment 202 where employees were installing drywall, the extension cord insulation was damaged through the outer insulation exposing interior wiring, thereby exposing employees to a contact hazard. b. In apartment 106 leading to the courtyard where an employee plugged in an extension cord, the ground prong was missing from the extension cord, thereby exposing employees to a contact hazard. Moore Paint and Drywall, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.404(b)(1)(iii)(C), which was contained in OSHA inspection number 605678, Citation number 1, Item number 1-a and was affirmed as a final order on 10/05/2012 with respect to a workplace located at 720 West Third Street, Columbus Ohio 43214.

In apartment 202, employees were using an extension cord with exposed interior wiring insulation at the plug due to a failure of the strain relief, thereby being exposed to a contact hazard.
