29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. On the upper level landing in area A where employees accessed the second floor, the guardrail did not extend the full width of the opening, leaving 14 inches of exposed area. This opening exposed employees to a fall hazard of 15 feet 4 inches.

29 CFR 1926.502(b)(2): Midrails, screens, mesh, intermediate vertical members, or equivalent intermediate structural members shall be installed between the top edge of the guardrail system and the walking/working surface when there is no wall or parapet wall at least 21 inches (53 cm) high. a. On the upper level landing in area A where employees accessed the second floor, there was no midrail installed, thereby exposing employees to a 15 feet 4 inche fall hazard.
