29 CFR 1926.25(a): During the course of construction, alteration, or repairs, form and scrap lumber with protruding nails, and all other debris, were not kept cleared from work areas, passageways, and stairs, in and around buildings or other structures. Throughout the facility; employees work areas and aisle/walkways contained excessive piles of furniture and other debris obstructing the employees walkways and work areas, exposing the employees to tripping hazards.

29 CFR 1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, were not protected by protective helmets. Mechanical room area; the employer did not provide head protection to employees who walked back and forth underneath a roof that had concrete masonry units and other debris at edge of the roof, exposing employees to struck-by hazards.

29 CFR 1926.1101(k)(7)(i): Warning signs that demarcate the regulated area were not provided and displayed at each location where a regulated area is required to be established by paragraph (e) of this section. Signs were not posted at such a distance from such a location that an employee may read the signs and take necessary protective steps before entering the area marked by the signs. At the facility; regulated areas with asbestos containing materials (ACM) were not properly marked with warning signs, exposing employees to contact/inhalation hazards.
