29 CFR 1910.147(c)(4)(ii)(B): The energy control procedures did not clearly and specifically outline the steps for shutting down, isolating, blocking and securing machines or equipment to control hazardous energy: (a) Press Room: On or about 24 January 2013 and at times prior, the employer had not developed a written, machine specific procedure to include specific procedural steps for shutting down and isolating all potential energy sources, including but not limited to the electrical and pneumatic, for the Goss Community Printing Press prior to the commencement of machine repair and maintenance operations.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: (a) Press room: On or about 24 January 2013 and at times prior, the lower print roller on the top printing tower of the Goss Community Press was not guarded exposing employees to the hazard of rotating parts. (b) Press room: On or about 24 January 2013 and at times prior, the cover was missing on the inlet to the stacking portion of the Mueller Martini Minuteman machine exposing employees to in going nip points. (c) Press room: On or about 24 January 2013 and at times prior, the inspection plate on the Mueller Martini Minuteman Saddle Stitcher was not attached exposing employees to the rotating parts. (d) Press room: On or about 24 January 2013 and at times prior, the cover over the Mueller Martini conveying area from the Minuteman Saddle Stitcher to the "Cone of Silence" was missing exposing employees to rotating parts.

29 CFR 1910.303(b)(2): Listed or labeled electrical equipment was not used or installed in accordance with instructions included in the listing or labeling: (a) Plate Room: On or about 24 January 2013 and at times prior, employees were exposed to a fire hazard by routing the power for the Computer Tube Plate (CTP) machine through a permanently wall mounted relocatable power tap which was plugged into a duplex receptacle.
