(c) Press room: On or about 24 January 2013 and at times prior, the inspection plate on the Mueller Martini Minuteman Saddle Stitcher was not attached exposing employees to the rotating parts.    (d) Press room: On or about 24 January 2013 and at times prior, the cover over the Mueller Martini conveying area from the Minuteman Saddle Stitcher to the "Cone of Silence" was missing exposing employees to rotating parts.
