29 CFR 1910.178(a)(4):     Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly.    a) The company did not get the approval from the manufacturer for modification, in that the company removed the back stop, exposing the slide cylinder, chains and sprocket to damage on the Yale sit down forklift, Model #155VX, S/N C879Vo1530E.

29 CFR 1910.178(e)(2):     If the type of load presents a hazard, the user shall equip fork trucks with a vertical load backrest extension manufactured in accordance with paragraph (a) (2) of this section.    a) The backrest was not attached to the Yale sit down forklift, Model # 155VX, s/n C879O1530E, to prevent contact with parts being lifted.

29 CFR 1910.178(q)(7):     Industrial trucks shall be examined before being placed in service, and shall not be placed in service if the examination shows any condition adversely affecting the safety of the vehicle. Such examination shall be made at least daily. Where industrial trucks are used on a round-the-clock basis, they shall be examined after each shift. Defects when found shall be immediately reported and corrected.    a) The Yale forklift Model # 155VX, s/n C879VO153OE, was not removed from service after defect(s) were found in that the back stops were removed and damage was present to a cylinder, bent and broken metal parts .

One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. 29 CFR 1910.219(c)(2)(i):     All exposed parts of horizontal shafting seven (7) feet or less from floor or working platform, excepting runways used exclusively for oiling, or running adjustments, shall be protected by a stationary casing enclosing shafting completely or by a trough enclosing sides and top or sides and bottom of shafting as location requires.    a)  The shaft attached to the motor drive on the Version Press, LP#1 was not guarded exposing employees to rotating parts.  b)  The shaft attached to the motor drive on the Version Press  LP#4 was not guarded exposing employees to rotating parts.

29 CFR 1910.303(b)(1):     Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees. Safety of equipment shall be determined using the following considerations:    a) Ground Fault Circuit Intterrupter(GFCI) receptacle in the Millport area,  was not inspected for defects in that the box was cracked in half.    b) Ground Fault Circuit Interrupter (GFCI) receptacle in the Millport area,  was not inspected for defects in that the box had metal shavings inside of the boxes.
