On or about January 18, 2013, protective equipment for head and extremities was not provided and used properly in that personal fall arrest equipment for employees accessing the evaporative condenser for Engine Room 3 was not provided with anchorages that were not being used to support or suspend platforms, and were capable of supporting at least 5, 000 pounds per employee.

29 CFR 1910.119(e)(3)(i): The process hazard analysis did not address the hazards of the process.     On or about January 18, 2013, the previous process hazard analysis for the anhydrous ammonia refrigeration system and its various components did not address the hazards of the process, including but not limited to, the following:     a) The Process Hazard Analysis for Engine Rooms 1, 2, 4 and 4b did not address the hazard of an anhydrous ammonia release in the engine rooms that involves employee contact with anhydrous ammonia and the need to utilize an emergency eye wash station and deluge body shower located external to the engine rooms due to ammonia concentrations and hazards within the engine room.     b) The Process Hazard Analysis for Engine Rooms 1, 2, 4 and 4b did not address the hazard of an anhydrous ammonia release in the engine rooms and notification to employees prior to entering the engine rooms.     c) The Process Hazard Analysis for Engine Room 2 did not address the hazard of an anhydrous ammonia release in the engine room and the need to employees to rapidly exit through an exit door that did not open outward.     d) The Process Hazard Analysis for Engine Rooms 1, 2, 4 and 4b did not address the hazard of an anhydrous ammonia release in the engine rooms and the need for emergency remote controls for the emergency mechanical ventilation system immediately outside the designated principle exterior engine room doors.      e) The Process Hazard Analysis for the anhydrous ammonia refrigeration system did not address the hazard of exposure to and contact with anhydrous ammonia at all ammonia handling points, such as but not limited to, the roof top evaporative condensers for Engine Rooms 1, 2, 4 and 4b.

29 CFR 1910.119(d)(3)(ii): The employer did not document that equipment complies with recognized and generally accepted good engineering practices.     a) Engine Rooms 1, and 4 did not have an emergency eye wash station and deluge body shower located external to the engine rooms and readily accessible, per ANSI/IIAR 2 - 2008 "Equipment, Design, and Installation of Closed-Circuit Ammonia Mechanical Refrigeration Systems" - Section 13.1.6.1.     b) Engine Rooms 1, 4 and 4b did not have an audible and visual alarm inside each refrigerating machine room and outside each entrance to the refrigerating machine room, per ANSI/IIAR 2 - 2008 "Equipment, Design, and Installation of Closed-Circuit Ammonia Mechanical Refrigeration Systems" - Section 13.2.1.2.     c) On the rooftop, the discharge location from multiple pressure safety valves to the atmosphere were not 15 feet or greater from the adjacent grade or roof level, and were not arranged to avoid spraying refrigerant on persons in the vicinity, ANSI/IIAR 2 - 2008 "Equipment, Design, and Installation of Closed-Circuit Ammonia Mechanical Refrigeration Systems" Section 11.3.6.4.     d) Irrigation facilities, and/or eye wash bottles were not available at all ammonia handling points, such as but not limited to, the roof top evaporative condensers for Engine Rooms 1, 2, 4 and 4b , IIAR Bulletin  - 2008 "Equipment, Design, and Installation of Closed-Circuit Ammonia Mechanical Refrigeration Systems" Section 13.1.11.1.
