29 CFR 1926.416(e)(1):  Worn or frayed electric cords or cables were used:         a)  On or about January 18, 2013; 6th floor area;  the employer, the general contractor for a large demolition project, did not ensure a safe working environment for all employees and sub-contractors when an employee was observed using a damaged portable extension cord that had the outer sheath damaged exposing the inner wiring conductors.       No abatement certification or documentation required.

29 CFR 1926.404(f)(6):  The path to ground from circuits, equipment, or enclosures was not permanent and continuous:      a) On January 18, 2013; 6th floor; the employer, the general contractor for a large demolition project, did not ensure a safe working environment for all employees and sub-contractors when an employee was observed using two extension cords that had an open ground. The extension cords were used to provide power to equipment such as but not limited to temporary lighting and a pipe cutting/threading machine.     No abatement certification or documentation required.

29 CFR 1910.134(c)(2)(i) as referenced by 29 CFR 1926.103:  Respirator users were not provided with the information contained in Appendix D to 29 CFR 1910.134 when the employer determined that any voluntary respirator use was permissible:        a) On or about January 18, 2013, throughout site; the employer, the general contractor on a large demolition project, did ensure that the sub-contractor employees were provided with the information in Appendix D of this standard prior to the voluntary use of N95 respirators.         Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
