29 CFR 1904.29(b)(3): Each recordable injury or illness was not entered on the OSHA 300 Log and/or an incident report (OSHA Form 301or equivalent) within seven (7) calendar days of receiving information that a recordable injury or illness has occurred: a) On or about January 24, 2013 and at times prior, the employer did not generate the OSHA 300 Log for the Year 2012, and did not record five (5) injuries or illnessses that had occurred. Non-recorded injuries include medical treatment provided to a warehousemen on November 29, 2012 for his left and right hands, a lumbar strain on November 19, 2012 for a driver and medical treatment provided to a driver for a sprain knee on August 8, 2012. Abatement Note: Abatement certification is required for this item. (see enclosed "Certification of Corrective Action" worksheet).
