(a) Location - Laboratory: On or about, 1/28/13, an adapter had been connected to a three prong plug so that it could be used with a two prong outlet. This removed the effective path to ground for that plug.

This includes not being trained on how to deal with high levels of hydrogen sulfide in the laboratory.
