29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a) Jobsite, 5980 University Boulevard, Coraopolis/Moon Township, PA: On or about January 18, 2013, an employee was in the process of installing shingles on a commercial roof and was not protected from falling. The pitch of the roof was 5 in 12 and the fall hazard was approximately 10 feet to the ground below.

29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed: a) Jobsite, 5980 University Boulevard, Coraopolis/Moon Township, PA: On or about January 18, 2013, an employee was using a 24 foot aluminum extension ladder to access the roof and this ladder was only extended approximately 6 inches above the roof. The fall hazard to the employee was about 10 feet to the ground below.

29 CFR 1926.503(c)(2): The employer did not update and re-train the employees on previous training that is now obsolete: a) Jobsite, 5980 University Boulevard, Coraopolis/Moon Township, PA: On or about January 18, 2013, an employee was working on a commercial roof without fall protection and had been previously trained in the requirements of fall protection by the employer, but incorrectly according to the "Jobsite General Safety Rules" provided by the employer. The safety rules included the statement, "If exposed to a fall you must be protected by slate catches or personal fall arrest sytem."
