29 CFR 1910.23(c)(1): Open-sided floors and/or platforms four feet or more above adjacent floor or ground level were not guarded with standard railings (or equivalent) and toeboards:    a) Mechanic(s) repair and service air compressor equipment located in the mechanical room above the soldering room in the back corner of the building; exposed to 11' fall hazard due to open-sided floor without railing or equivalent fall protection.      b) Mechanic(s) accessed the upper mechanical room housing the air compressors via personnel platform elevated by forklift truck where the front side of the cage was left unguarded by rail, chain or similar barrier; exposing employee(s) to falls of up to 11'.        Note:  Toeboards are only required when persons can pass, there is moving equipment or there is equipment with which falling materials could create a hazard.

Each respirator user was not provided a medical evaluation to ensure their physical / medical suitability to wear negative-pressure type respirator(s).    Condition(s) were noted on or about 5/2/2013.    Note:  The employer may discontinue an employee's medical evaluations when the employee is no longer required to use a respirator.

29 CFR 1910.215(b)(9): Guard for abrasive wheel machine where the operator stands in front of the machine was not constructed so that the peripheral protecting member could be adjusted to the constantly decreasing diameter of the wheel:    a) The larger (Cincinnati brand) pedestal grinder was used by machinist(s) for grinding metal parts of motor armatures without peripheral guard(s) in place and adjusted to within one-quarter inch of the wheel perimeter.  Machinist(s) operating the larger pedestal grinder without the tongue guard in place were exposed to injury from flying wheel fragments in the event of rupture of the abrasive wheel during use.    Condition(s) were noted on or about 1/28/2013.
