29 CFR 1926.760(b)(3): Be provided, at heights over 15 and up to 30 feet above a lower level, with a personal fall arrest system, positioning device system or fall restraint system and wear the equipment necessary to be able to be tied off; or be provided with other means of protection from fall hazards in accordance with paragraph (a)(1) of this section. Jobsite- On or about January 24, 2013 employees were connecting steel trusses without fall protection at a height of 17 feet.

29 CFR 1926.1402(b): The equipment must not be assembled or used unless ground conditions are firm, drained, and graded to a sufficient extent so that, in conjunction (if necessary) with the use of supporting materials, the equipment manufacturer's specifications for adequate support and degree of level of the equipment are met. The requirement for the ground to be drained does not apply to marshes/wetlands. Jobsite - On or about January 24, 2013 employees were using a International 4200 VT365 truck with a Elliott 1870F boom truck crane to locate steel trusses to be welded. The outriggers and cribbing were sunken into the muddy ground.
