29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a) On the north side of the Gray-Line Winder - On or about January 3, 2013, employees, operating the roll winder, were not protected from the unguarded ingoing nip points and rotating parts of the winder where the stiff plastic sheet was wound onto the roll cores. b) At the plastic extrusion machines in the Profile Department - On or about January 24, 2013, the employees, working around the extruders, were not protected from the heat at the point of operation of the unguarded 300-degree hot plastic extruders. No abatement certification or documentation required.

29 CFR 1910.1030(f)(2)(i): Hepatitis B vaccination was not made available after the employee had received the training required in 29 CFR 1910.1030(g)(2)(vii)(I) or within 10 working days of initial assignment to employees who had occupational exposure: a) On the north side of the Gray-Line Winder - On or about August 17, 2012, the employer had not offered hepatitis B vaccinations within 10 days of initial assignment to the emergency response team where employees had a reasonable expectation of occupational exposure to blood while providing life-saving first aid during an amputation incident. Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and states employees and their representatives were informed of this abatement.
