29 CFR 1926.701(b): All protruding reinforcing steel, onto and/or into which employees could fall or come against, was not guarded to eliminate the hazard of impalement: (a) 1201 Science Park Drive site, South Charleston, WV - Prior to concrete being poured into forms on the ground employees were exposed to an impalement hazard in that several slab form pins were sticking seven inches above the wooden forms they were attached to and they were not protected with caps as observed on January 29, 2013.
