29 CFR 1910.1200(f)(9):     The employer shall ensure that labels or other forms of warning are legible, in English, and prominently displayed on the container, or readily available in the work area throughout each work shift. Employers having employees who speak other languages may add the information in their language to the material presented, as long as the information is presented in English as well.    (a) Location - Indian Ridge, Westford, MA:    On or about 1/28/13, the employer failed to ensure that LP gas containers were appropriately labeled.

29 CFR 1926.153(h)(11):     Containers having a water capacity greater than 2 1/2 pounds (nominal 1 pound LP-Gas capacity) connected for use shall stand on a firm and substantially level surface and, when necessary, shall be secured in an upright position.    (a) Location - Indian Ridge, Westford, MA:    On or about 1/28/13, the employer failed to secure a 100-pound LP gas container while it was in use.

29 CFR 1926.153(h)(13):     For temporary heating, heaters (other than integral heater-container units) shall be located at least 6 feet from any LP-Gas container. This shall not prohibit the use of heaters specifically designed for attachment to the container or to a supporting standard, provided they are designed and installed so as to prevent direct or radiant heat application from the heater onto the containers. Blower and radiant type heaters shall not be directed toward any LP-Gas container within 20 feet.    (a) Location - Indian Ridge, Westford, MA:    On or about 1/28/13, the employer failed to ensure that a space heater was at least 6 feet from a LP gas container.

29 CFR 1926.404(b)(1)(ii):     Ground-fault circuit interrupters. All 120-volt, single-phase 15- and 20-ampere receptacle outlets on construction sites, which are not a part of the permanent wiring of the building or structure and which are in use by employees, shall have approved ground-fault circuit interrupters for personnel protection. Receptacles on a two-wire, single-phase portable or vehicle-mounted generator rated not more than 5kW, where the circuit conductors of the generator are insulated from the generator frame and all other grounded surfaces, need not be protected with ground-fault circuit interrupters.    (a) Location - Indian Ridge, Westford, MA:    On or about 1/28/12, the employer failed to provide ground-fault circuit protection for emloyees using a 6500-watt generator.
