OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to the hazards of asphyxiation and delayed rescue from a confined space: (a) Location - Indian Ridge, Westford, MA: On or about 1/28/13, employees entered a man hole meeting the requirements of a permit required confined space while oxygen levels in the man hole were not evaluated to determine if entry would be safe. (b) Location - Indian Ridge, Westford, MA: On or about 1/28/13, employees entered a man hole meeting the requirements of a permit required confined space and were not using a chest or full body harness to facilitate non-entry rescue. (c) Location - Indian Ridge, Westford, MA: On or about 1/28/13, employees entered a man hole meeting the requirements of a permit required confined space and were not using a retrieval device to facilitate non-entry rescue.
