29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:    (a)  On or about January 29, 2013,  employees perform servicing and maintenance on various machinery and equipment (i.e. welders, a horizontal band saw, CNC milling machines, and metal lathes) and the machinery and equipment was not locked out.  The employer did not document and implement an energy control program to protect employees from accidental and uninteneded startup of machinery.     Abatement documentation is required for this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1910.212(a)(3)(ii):     Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):    a)On or about January 29, 2013, employees were exposed to amputation hazards from an unguarded segment of a band saw next to the shipping and receiving section of H.A. Phillips manufacturing facility.     b) On or about January 29, 2013, employees were exposed to amputation hazards from a CNC mill that had a bypassed interlock device that was disconnected from the door in the valve department. The employer did not properly install the interlock device on the door of the CNC mill, allowing the mill to run while the door remained open.     There is no abatement certification or documentation required for this item.
