29 CFR 1926.1417(o)(3)(i): The weight of the load was not determined from a source recognized by the industry (such as the load�s manufacturer), or by a calculation method recognized by the industry (such as calculating a steel beam from measured dimensions and a known per foot weight), or by other equally reliable means: The employer is failing to protect employees from overhead struck-by and crane collapse hazards. On January 29th, 2013 the employer over loaded a crawler crane after the load to be lifted exceeded the cranes load limit causing the crane to collapse. The operator was pinned in the cab and 2 workers that were on a concrete delivery pipe were thrown into the Missouri River. Calculations for the load were not based on recognized source. This then caused the operator to exceed the maximum load rating of the crane.
