29 CFR 1910.305(g)(2)(iii): Flexible cords were not connected to devices and fittings so that tension would not be transmitted to joints or terminal screws: In the Compounding / Mixing Room: The employer did not ensure that flexible cords used as pendants were provided with adequate strain relief. On the roll forming machine ( unknown mfg) the flexible cord energizing the machine was wrapped around an arm of the machine to prevent it from disconnecting.
